LTAC Diagnoses and Treatments for Referring Physicians

LTAC hospitals provide diagnostic tests and medical treatment to patients with chronic diseases and medically complex conditions. LTAC hospital patients are too ill for placement in skilled nursing home.

Roosevelt follows InterQual criteria to determine the appropriateness of patient admission and continued hospital stay. Daily physician rounds are required. Registered nurses certified in Advanced Cardiac Life Support (ACLS) are on duty twenty-four hours daily.  Respiratory, physical, occupational, and speech therapies are available along with registered dietitians, clinical pharmacist and case managers.

The six major InterQual subsets treated are: ventilator weaning, respiratory complex, other medically complex, infectious diseases, wound/skin, and cardiovascular/peripheral vascular.

RWSHL provides a Joint Commission certified Wound Care Program and Vent Weaning Program.

Early Identifiers:

  • Is the patient in ICU?
  • Respiratory failure?
  • Does patient have multi-system failure?
  • Does the patient have complex wound care needs?
  • Is the patient too medically unstable to be transferred to a lower level of care?
  • Is the patient’s rehabilitation compromised by acute medical issues?
  • Does the patient have a history of multiple re-admissions to acute care or at risk for medical complications when discharged?
  • Would patient benefit by daily physician visits and 24-hour physician coverage?
  • Does the patient have any of the following requirements &/or conditions?
    • Dialysis
    • Vent Weaning
    • Extensive wound care
    • Long term IV therapy
    • MRSA/VRE
    • TPN
    • Tracheotomy care/extensive respiratory care

To make a referral or for more information, please contact our admissions department at 706-655-5257 or fax information to 706-655-5258. You will be put in contact with one of our nurse liaisons to perform a pre-admission screening and coordinate transfer to our hospital.
